Here at Cloud, we're on a mission to deliver an entirely different primary care experience for you and your family. In order for your first appointment to be as effective as possible, we ask that you complete a New Patient Packet that outlines your medical history and current health goals.

This form gives us a broad overview of your medical history and current health goals. This may take anywhere from 15-30 minutes to complete.
If you have any recent and relevant medical records that you would like us to have in your patient chart with us, (ex. blood work results, imaging results).
You can email those records to [email protected].
Alternatively, we can collect these records on your behalf, however, this does tend to take longer, and is often quicker and easier for you to collect your previous records yourself. If you would like us to gather these records for you, please fill out and sign the following release form to allow us to collect your records.

How do I schedule my first appointment? Once we have received your New Patient Paperwork, we will give you a call to schedule you for your Meet and Greet appointment with one of our providers.
Will this appointment be in person or virtual? Either one! We can schedule you for an in office visit at any of our locations (Longmont, Boulder Peloton, or Boulder City Club) or virtually, via phone or Zoom video.
Who will I meet with for this first appointment? All four of our clinicians are fantastic to meet with for your Meet and Greet appointment. During this 30 minute appointment, the two of you will discuss your health goals and concerns and lay out the next steps in your plan of care. In order for this appointment to be as effective as possible, please be as detailed as possible when completing your new patient packet that outlines your medical history your current health goals.

We will meet you where you are - no need to take 2 hours off of work to come into the office to bring in your sick kiddo. We offer you the care that you need virtually!
We act as your urgent care clinic as well. We reserve same-day appointment slots for acute issues, so if we do need to see you in person for something like stitches, we will!
